Patient Registration​

To become a medical cannabis patient, you must be diagnosed with a "serious medical condition" and be certified by a registered physicianAfter recieving a certfication, patients must register through the online application portal and submit an application and application fee ($50) to the Office of Medical Cannabis (OMC). Applications are then reviewed by the OMC within 30 days. 

Updates regarding the application status will be sent via email. If approved, patients will revisit the application portal and download their digital medical cannabis card. Applications that are rejected for corrections may be fixed and resubmitted to the OMC for review.​

If mailing a check or money order: 

​                 (a) ​make payable to West Virginia Department of Health 

                 (b) include the application reference number given after submitting the application

                 (c) mail to 350 Capitol St, Room 523, Charleston, WV 25301







Your application will be reviewed within 30 days. After review, you will receive an email update about the status of your application. 

 If your application is approved, you will be notified via email to login and download your approved OMC Medical ID Card.

 If your application needs a correction, it will be rejected and sent back to you to correct and resubmit.You will not need to create a new application, only make the corrections. If your application is denied, you will be informed of the reason via email. ​

​​Registration - Frequently Asked Questions


Physicians who have completed a mandatory four-hour physician education course and are registered with the Office of Medical Cannabis (OMC) can certify patients for medical cannabis. Registered physcians will submit your completed certication form directly to the OMC. 

NOTE: Registering for a physicians appointment is separate from applying and registering for a patient card with the OMC. Patients should NOT email or mail the Patient Certification form to the OMC.

To schedule a telehealth appointment, vist the "Schedule an Appointment" page to find the company that is best for you. 


When completing the registration application through MyComplia, patients will need to upload the following: 

  • Digital photo of passport quality
  • Photo ID, such as a copy of your driver’s license, passport, or state ID. 
  • Proof of West Virginia residency. View the list of acceptable documentation here​
  • Physician Certification
NOTE: If requesting a fee waiver​ based on financial hardship (income below 200% of the federal poverty level), provide acceptable proof of income, such as W-2, recent pay stubs, proof of benefit eligibility, etc. Please do not send payment if you are requesting the fee waiver. If the waiver is not granted, you will be notified to submit the fee at that time. ​

After approval, a medical cannabis patient card will expire after one year. If a patient decides they want to continue to participate in the medical cannabis program, they must revisit with a registered physician and submit a renewal application.  

Patients must renew ANNUALLY.​​


Typically applications are rejected back to the applicant for application corrections. If your application needs a correction(s) you will be notified of what corrections are needed via email and the registration portal. You will not need to create a new application, only make the corrections. Any status updates on your application will be sent via email.  

Common rejection reasons include: 
  • blurred, cropped, or filtered digital photos 
  • expired Driver's License or State ID
  • Missing documents or payment 
  • Invalid online-electronic physician certification ​​

Serious Medical Conditions recognized by the State of West Virginia: 

  • ​Cancer 
  • Position statucs for human immunodeficiency virus (HIV) or acquired immune deficiency syndrome (AIDS) 
  • Amyotrophic lateral sclerosis (ALS)
  • Parkinson's disease 
  • Multiple sclerosis (MS) 
  • Damage to the nervous tissue of the spinal cord with objective neurological indictation of intractable spasticity. 
  • Epilepsy
  • Neuropathies 
  • Huntington's disease 
  • Crohn's disease 
  • Post-traumatic stress disorder 
  • Intractable seizures 
  • Sickle cell anemia 
  • Severe chronic or intractable pain of neuropathic origin or severe chronic or intractable pain 
  • Terminal illness that is defined as a medical prognosis of life expectancy of approximately one year or less if the illness runs its normal course.
