Adverse Event Reporting

​​​​​​​​​​​​​​​An adverse event is defined in W. Va. Code R §64-110-9 as an injury resulting from the use of medical cannabis dispensed at a dispensary. An injury includes physical harm, mental harm, or loss of function. If you or someone you know has experienced an adverse event related to medical cannabis or a medical cannabis product, please fill out the form below to report the adverse event to the OMC.

Adverse event reporting is critical in monitoring the safety of medical cannabis products and can help the OMC and medical cannabis industry permittees to identify and respond to possible health and safety issues with medical cannabis and/or medical cannabis products. The OMC is committed to ensuring the health and safety of medical cannabis patients participating in the medical cannabis program and monitoring adverse events will help the OMC uphold their commitment.

Please note: This form is to collect information about adverse events experienced by patients ONLY.  If you are a patient reporting, please be honest about your experience.​​ If you wish to report anonymously, please write "N/A" in the personal information areas of the form. 


Complaints related to the medical cannabis program, the OMC, patients, or permittees should be submitted through the OMC Comment Form found on the homepage of the OMC website. ​​
