This event is hosted by Cannabist Dispensaries. Releaf Specialists, telehealth physician will be available for those in need of evaluation and certification. The Office of Medical Cannabis will be on-site to assist patients complete the electronic Patient Identification Card application.
Documents that will be required are: Photo ID, i.e. driver's license; Proof of WV Residency, i.e. voter registration, vehicle registration, utility bill with name and physical address, etc. and the Physician's Certification for Qualified Patients.
Physician fee is $125.00 paid by debit/credit card.
Application processing fee is $50.00, paid by check or money order only. If you want to request a waiver of the $50.00 fee you will need to provide proof of hardship, i.e. a current benefit letter with your name and amount of benefits received, current W2s or paystubs.
